At Smart Health Ahead Inc we respect your privacy and appreciate the trust you have placed in us. This Privacy Policy explains how (“Smart Health Ahead”, “we”, “our”, and “us”) collects, uses, and discloses information we obtain through www.apticity.io 

 (the “Website“) and the mobile applications we support (“Apps”) that link to this Privacy Policy (collectively, the “Services”). This Privacy Policy is incorporated by reference into Smart Health Ahead Inc Terms of Service (the “Terms”). By agreeing to this Privacy Policy in your account setup or by using the Services, you consent to the processing of your information as set forth in this Privacy Policy.

In providing the Apps, we may act as a service provider to health care providers or payers you have a direct relationship with (“Health Practices”). These Health Practices have elected to utilize our Apps, scanners, and other products to support patient treatment. To the extent that any of our Apps are being offered on behalf of these Health Practices, the information that we collect from and about you (as described in this Privacy Policy) may be shared with these Health Practices to support the care and services they offer.

HOW WE COLLECT INFORMATION

Information You Provide

We collect any information you provide to us when you use the Services.

  • Account Registration. When you register an account with us, we request that you provide your name and email address. In addition, you are required to provide additional information about yourself including your height, gender, weight, and date of birth etc.
  • Smart Health Ahead Apticity App. We collect any information that you provide when you upload readings or go through the tests as set in Apticity Apps. For example, users using our Apticity app are able to upload over Eighty health metrics to our Apps such as their Vision test, Hearing test, blood pressure, Information bout their environment, previous medical conditions, etc
  • Health Practice Submitted Information. Health Practices like Care Providers Care Givers may provide us with information about their seniors or patients through the Services. These Health Practices are responsible for providing and obtaining all required notices and consents for us to process this information.
  • Communications and Other Interactions. We may collect information through your communications with our customer-support team. We will also collect any information you provide when requesting additional information about our services such as your name, phone number, mailing address, and email address.
  • . If you make payments through the Services, we will ask you to provide your payment information, such as your credit card number, so our service providers can process your payments

Information We Automatically Collect through the Services

We may automatically collect certain technical information from your computer or mobile device when you use the Services.

  • Device and Usage Information. We receive information from your devices, including IP address, web browser type, operating system version, and unique device identifiers. We may also collect and store your browser settings and preferences. We receive information about your interactions with our Services, such as the time spent interacting with content and the dates and times of visits.

In addition, to the extent our Services are being offered via a Health Practice that has a direct relationship with you, the information collected will be used to support the care and services the relevant Health Practice offers.

We use the information collected and provided to us to communicate with you. For example, we may send emails to the email address provided to verify an account and for informational and operational purposes, such as account management or customer service. To the extent permitted under applicable law, we may use the information we collect from and about you to provide regarding the Services that may be useful, relevant, valuable or otherwise of interest.

Compliance with applicable law and our own obligations

We may also process the information we collect for the following purposes: (i) to enforce our Terms or other legal rights, including intellectual property infringement; (ii) as may be required by applicable laws and regulations or requested by any judicial process or governmental agency; and (iii) comply with industry standards and our policies.

Aggregate Data

We may de-identify your information and process it in an anonymous and/or aggregated form for other purposes than described above.

Service Providers

We work with third party service providers who provide web development, hosting services, as well as other business services for us. To the extent it is necessary for these service providers to receive your information in order for us to provide the Services; these third parties may have access to or process your information.

Analytics and Ad Networks

We use third party analytics vendors to evaluate and provide us with information about your use of the Services. We do not share your information with these third parties, but theses analytics service providers may set and access their own cookies, pixel tags and similar technologies, and they may otherwise collect or have access to information about you which they may collect over time and across different websites. Browsing on and interactions with that property, is subject to that property’s own terms and policies. The NPP is intended to provide notice on how the covered entity may use and share protected health information and inform patients about their health privacy rights.

CHILDREN’S PRIVACY

We do not knowingly collect information online from children under the age of 13. If Smart Health Ahead Inc becomes aware that any person submitting information to the Services is under the age of 13, we will promptly take steps to delete any personal information collected. If you are visiting from the European Economic Area, Asia-Pacific region, or other regions with laws governing data collection and use should be aware that we may transfer your information to recipients in countries other than the country in which the information was originally collected, including the United States of America.
